I was seating a wire in a wall phone base using a knife edge years ago. They make a little tool for the insulation displacement 'slot' but did I stop and get it? Nooooo...
About a 1/2 a second before it happened, I was pressing the knife down in the slot and holding the base with my other hand, and the thought went through my mind "This is stupid.."
Yep, knife slipped out of the slot and straight down into the web between thumb and forefinger. Blade penetrated probably an inch into the inside of the palm of my hand....
I yelled at my nurse sister in law, (whom I was fixing the phone plate for,) that I needed to take a run to the ER where she worked which was only about 2 blocks away.
Wife found me at the ER an hour later. (Way before cell phone days..)
I asked her how she knew where I was, and she replied 'I saw the phone plate hanging out of the wall from the wires and the blood on the floor of the kitchen...'
