Sounds like Massachusetts, which has an amazing number of "dangerous roads" and "dangerous intersections". They get described in ways similar to what you said.
It seems to me all that can be summarized as "inadequate or nonexistent enforcement". One wonders why. It isn't hard to enforce traffic laws, and inflict appropriate pain on offenders especially repeat ones. But some states don't seem to get it.
I can attest to the non-existent quality of drivers in MA. There seems to be one overall factor regarding MVA's; the lack of attention paid to the task at hand.

The company van I drive is limited to speed limits or under. all other drivers want to drive 15-30 mph OVER the posted speed. they catch up and sit 5 feet from your ass. sometimes you cant see them in your outside mirrors. everyone comments on crash articles “ they were probably speeding/tailgating/distracted” . then they go and do all 3. its normal, everyone does it , my brakes are as good as thiers…….. CRASH. the stupid is deep.

The state needs to flood that stretch with cops and have them get nit-picking on the law. 7 over the limit? ticket. following too close? ticket. riding/blocking the left lane? ticket. unsignaled lane change? ticket. On and on and on. The cops would have a ball and make lots of money for the state.
