CoreCivic released the following statement to WSMV4:
“On April 22, 2025, at approximately 3 p.m., a correctional officer assisting in the transport of an inmate to the Meharry Medical College of Dentistry reported leaving his firearm in a restroom. Campus Safety and Security officials were notified. Meharry’s security staff evacuated the facility to conduct a thorough search, but no weapon was found. The officer involved has been placed on administrative leave while the incident is being investigated.”

I have been carrying for over a quarter of a century and had to attend to my bodily functions outside the house while armed numerous times. Each and every time I paid equal attention to my disposal of bodily fluids and solids and the need to keep my firearm under my strict control.
I bet the officer did NOT forget the phone because he was grossly involved with it.
"grossly involved" can have a number of meanings in the context of this situation ... Just sayin'.

I saw a public service announcement/poster/ad one time saying that if you put something important in the back seat of your car, you will not forget your child is there.

Something important? So you do not forget your child?
Seriously though. We all go into autopilot sometimes. And, if you have your brief case next to the child you were supposed to drop off a day care, you will forget neither. But have it sitting on the front passenger seat and next thing you know you are at work. Same with carrying a firearm.
The talent/trick is to avoid autopilot. (Which you should NEVER be on if you are carrying... that is condition white.) Too bad security guards, and all too often cops are all too human, and their job generally consists of boredom interrupted by occasional paperwork.
